Getting Started 1. Call our inquiry line (720)257-7811 to find out how our program works and if you are eligible. 2. Fill out the Patient Application (mail, e-mail, fax, download) and gather all the financial paperwork it asks for. 3. Once you’ve gathered all the documents, call Tasha (720)257-7802 to set up a Group Information intake appointment.4. Bring all your paperwork to the Group Information Session. During this meeting you will learn everything you need to know about the program. You will also be scheduled for a Financial Intake appointment. 5. Your Financial Intake appointment will determine your copay amount based on a sliding fee scale of your individual financial situation. If you have brought in all your paperwork and are eligible, you will be added to our program. |
